"Lead Off" alarm continuously aa raha hai aur ECG waveform nahi dikh rahi.

Electrode properly attached nahi

Electrode gel dry ho gaya

Patient ki skin oily ya sweaty hai

Lead wire damaged

Wrong lead configuration

All electrode placements check karein

Electrode connections lead wires se check karein

Lead wire cable inspect karein

Patient skin condition check karein

Electrodes remove karein, skin clean karein, fresh electrodes apply karein

Skin preparation: alcohol wipe se clean karein, dry karein, then electrode apply

Lead wires firmly snap onto electrodes

Damaged lead wire replace karein

Correct lead placement verify karein (RA, LA, LL, RL, V1-V6)

ECG Lead Off ka matlab patient danger mein nahi hai — monitoring gap hai. Quickly reconnect karein.
